Why Injections?
Testosterone injections are the most widely prescribed form of TRT, used by approximately 70% of men on therapy. They offer the best combination of effectiveness, cost, and dosing control.
Unlike topical methods, injections deliver a precise dose directly into the body with no risk of transfer to partners, children, or pets.
Types of Injectable Testosterone
- Testosterone Cypionate (Depo-Testosterone) — The most commonly prescribed in the US. Oil-based, with a half-life of 8-12 days. Typically injected weekly or every other week.
- Testosterone Enanthate (Delatestryl) — Very similar to cypionate. Slightly shorter half-life. Popular in the US and Europe.
- Testosterone Propionate — Shorter-acting (2-3 day half-life). Requires more frequent injections but provides very stable levels.
- Testosterone Undecanoate (Aveed) — Long-acting injection given every 10 weeks. Administered in-office only.
Injection Methods
- Intramuscular (IM) — Injected into the muscle (glutes, quads, delts). Uses a 1-1.5" needle. Most traditional method.
- Subcutaneous (SubQ) — Injected into fat tissue (abdomen, love handles). Uses a small insulin needle (½-⅝"). Increasingly popular for its ease and less pain.
Many clinics now recommend subcutaneous injections for their convenience and more stable absorption rates.
Typical Protocol
- Dose: 100-200mg per week (individualized based on blood work)
- Frequency: 1-2 times per week for most stable levels
- Needle size: 25-29 gauge (thin, minimal pain)
- Self-injection: Most patients learn to inject at home after initial training
Your clinic will teach you proper injection technique and provide supplies. Most men become comfortable with self-injection within 2-3 tries.
What to Expect
After starting testosterone injections:
- Week 1-2: Initial mood and energy improvements
- Week 3-4: Libido begins to increase
- Week 4-8: Body composition changes start (less fat, more muscle tone)
- Month 3-6: Full effects realized — energy, mood, sexual function, body composition
