Five years ago, almost every TRT patient saw their doctor in an office. Today, online telehealth clinics handle a large and growing share of testosterone prescriptions in the United States. Both models work, but they suit different patients and different situations. Here is how to choose.
The Case for Online TRT Clinics
Telehealth TRT providers like Hims, Hone, Defy Medical, and others have built efficient platforms around a clear premise: most testosterone replacement is straightforward enough to manage remotely (NRG Clinic, 2026; Innerbody, 2026). The advantages are real.
Convenience. You can complete the entire intake, lab work, and medication delivery without leaving your home. Lab orders are sent to a Quest or LabCorp near you. Medications ship to your door.
Speed. Many online clinics can complete intake, labs, and a first prescription within one to two weeks. Booking an in person endocrinologist can take months in some markets.
Predictable pricing. Most telehealth clinics charge a flat monthly fee that includes consultation, medication, and routine follow ups. There are no surprise bills.
Comfort with the topic. Some patients find it easier to discuss sexual symptoms or low libido with a provider over a screen than in a busy clinic.
The Case for In Person Care
In person providers, including primary care doctors, endocrinologists, urologists, and dedicated men's health clinics, still offer advantages that telehealth cannot fully replace (SynergenX Health).
Hands on physical exams. A proper physical exam includes a prostate check, testicular exam, and assessment for signs of estrogen excess or other hormonal issues. Telehealth providers skip much of this.
Coordination with other care. If you have other conditions, such as heart disease, sleep apnea, or fertility goals, an in person provider can coordinate more effectively with your other specialists.
Insurance coverage. Most in person care runs through insurance, while telehealth concierge memberships are often cash pay.
Complex cases. If your hypogonadism is secondary, your prolactin is high, your fertility matters, or you have a history of prostate concerns, an in person specialist is the better fit.
When Online TRT Makes Sense
Telehealth is often a good choice if:
You have classic symptoms and likely primary or age related hypogonadism without complicating factors.
You are comfortable self injecting or using a gel and want a simple, fast care path.
You live in a rural area with limited access to endocrinologists or men's health specialists.
You have already been on TRT and just need straightforward maintenance and refills.
You value convenience and predictable cash pricing over insurance coverage.
When In Person Care Makes More Sense
In person care is usually the better fit if:
You want to use insurance and minimize out of pocket costs.
Your case is complex, including secondary hypogonadism, high prolactin, or coexisting health issues.
You are concerned about fertility and want to coordinate with a reproductive urologist.
You have a history of prostate cancer, severe sleep apnea, or recent cardiovascular events.
You prefer hands on physical exams and a provider you can see in person if questions arise.
