Testicular shrinkage is one of the most common and predictable side effects of testosterone replacement therapy. It happens because external testosterone shuts down the body's own signal to produce testosterone and sperm, and the testes shrink from disuse rather than from any direct damage. Most men lose some volume within the first few months of treatment, but the change is usually manageable and often reversible once it's addressed.
Why Does TRT Cause Testicular Shrinkage?
The testicles normally run on a feedback loop called the hypothalamic-pituitary-gonadal axis. The brain releases luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which signal the testes to produce testosterone and sperm. When testosterone is supplied from an outside source, the brain senses plenty of testosterone already in circulation and cuts back on LH and FSH (Desai et al., Therapeutic Advances in Urology, 2022).
Without that LH signal, the testes stop producing their own testosterone and largely stop producing sperm. Since testicular volume is driven mostly by active sperm production rather than by testosterone output itself, the loss of that signal is what causes the physical shrinkage, not any toxic effect of testosterone on testicular tissue.
How Much Do Testicles Actually Shrink on TRT?
The amount of shrinkage varies a lot between men, but it follows a fairly predictable pattern. A 2022 study that tracked 121 men on TRT for up to 24 months found average testicular volume dropped from 16.5 mL before treatment to 13.7 mL at the two-year mark, with the decline becoming statistically significant by six months (Journal of Sexual Medicine, 2022). That works out to roughly a 15 to 20 percent reduction in size for the average patient, though some men notice less and others notice considerably more.
Delivery method and dose play a role, since higher and steadier testosterone exposure suppresses LH more completely. Men on long-term, higher-dose protocols tend to see more shrinkage than men on lower, carefully titrated doses.
Is Testicular Shrinkage on TRT Reversible?
For most men, yes. Once testosterone therapy stops, the pituitary gradually resumes sending LH and FSH signals, and the testes typically regain most of their size within 6 to 12 months. Sperm production follows a similar timeline, though it can take up to two years in some cases, and about 70 percent of men who stop testosterone or anabolic steroid use recover meaningful sperm counts within a year (Desai et al., Therapeutic Advances in Urology, 2022). Older age and longer duration of testosterone use both predict a slower recovery, which is one more reason to address shrinkage early rather than waiting years to see if it resolves on its own.
Does HCG Prevent Testicular Shrinkage?
Human chorionic gonadotropin (HCG) mimics LH closely enough that it can keep the testes active even while the brain's own LH signal is suppressed by TRT. A controlled study in healthy men found that low-dose HCG, given alongside testosterone, maintained intratesticular testosterone at or above baseline levels, directly counteracting the shutdown that causes shrinkage (Coviello et al., Journal of Clinical Endocrinology & Metabolism, 2005).
The fertility data backs this up. In a study of men on TRT who added concomitant HCG, none developed azoospermia (a complete absence of sperm), and several went on to conceive with a partner during the study period (Hsieh et al., Journal of Urology, 2013). Most clinics that prescribe HCG alongside TRT use it two to three times per week by subcutaneous injection, and it can be started at the outset of therapy or added later if shrinkage or fertility concerns come up.
